We all want the best care for our aging family members and loved ones so they can enjoy their golden years to the fullest.
With a multitude of services available and doctors involved, ensuring loved ones are getting the right care can be confusing. People often think a nursing home or other facility is the answer, despite the seniors’ desire to stay in familiar surroundings. But we often struggle with leaving them at home alone without the supervision and companionship they want and need.
At Baptist Health PACE, we provide and coordinate all the medical care, in-home care, therapies, transportation, and other services your loved ones need to live a healthy, safe life in their own home.
Upon enrollment, participants receive all of their health care services through Baptist Health PACE, including:
Services covered are in accordance with the approved individualized care plan developed with your Baptist Health PACE care team.
Once enrolled in Baptist Health PACE, the amount you pay each month will not change no matter what care and services you might need.
If you have Medicaid, you won’t pay a monthly premium for the long-term care portion of the Baptist Health PACE benefit. If you don’t qualify for Medicaid, but you have Medicare, you’ll be charged these:
There’s no deductible or copayment for any drug, service, or care approved by your health care team. If you don’t have Medicare or Medicaid, you can pay for Baptist Health PACE privately.
Because Baptist Health PACE provides and is responsible for all of your care, you may be financially responsible for any care you receive outside the program not approved by the Baptist Health PACE interdisciplinary team.
